Drop in and see us (6:00 am - 6:30 pm) Monday thru Friday
to pick up enrollment packet. Enrollment forms require:
- Emergency contact numbers
- Those that are
allowed to pick up your child
- Physical forms
- Child's personal
information
Before your child may attend, we will need completed enrollment
form, immunization record, physical forms (must be returned within
30 days of enrollment), and paid enrollment fee. For those receiving
state assistance, we require proof of state approval.
